There have been few empirical studies into what non-medical factors influence physicians and nurses when deciding about admission and discharge of ICU patients. Information about the attitudes of healthcare professionals about this process can be used to improve decision-making about resource allocation in intensive care. To provide insight into ethical problems that influence the ICU admission and discharge process, we aimed to identify and explore ethical dilemmas healthcare professionals are faced with.

Patients with a life-threatening illness can be confronted with various types of loneliness, one of which is existential loneliness (EL). Since the experience of EL is extremely disruptive, the issue of EL is relevant for the practice of end-of-life care. Still, the literature on EL has generated little discussion and empirical substantiation and has never been systematically reviewed. In order to systematically review the literature, we (1) identified the existential loneliness literature; (2) established an organising framework for the review; (3) (...) conducted a conceptual analysis of existential loneliness; and (4) discussed its relevance for end-of-life care. We found that the EL concept is profoundly unclear. Distinguishing between three dimensions of EL—as a condition, as an experience, and as a process of inner growth—leads to some conceptual clarification. Analysis of these dimensions on the basis of their respective key notions—everpresent, feeling, defence; death, awareness, difficult communication; and inner growth, giving meaning, authenticity—further clarifies the concept. Although none of the key notions are unambiguous, they may function as a starting point for the development of care strategies on EL at the end of life. (shrink)

In their 2007 paper, Swierstra and Rip identify characteristic tropes and patterns of moral argumentation in the debate about the ethics of new and emerging science and technologies (or “NEST-ethics”). Taking their NEST-ethics structure as a starting point, we considered the debate about tissue engineering (TE), and argue what aspects we think ought to be a part of a rich and high-quality debate of TE. The debate surrounding TE seems to be predominantly a debate among experts. When considering the NEST-ethics (...) arguments that deal directly with technology, we can generally conclude that consequentialist arguments are by far the most prominently featured in discussions of TE. In addition, many papers discuss principles, rights and duties relevant to aspects of TE, both in a positive and in a critical sense. Justice arguments are only sporadically made, some “good life” arguments are used, others less so (such as the explicit articulation of perceived limits, or the technology as a technological fix for a social problem). Missing topics in the discussion, at least from the perspective of NEST-ethics, are second “level” arguments—those referring to techno-moral change connected to tissue engineering. Currently, the discussion about tissue engineering mostly focuses on its so-called “hard impacts”—quantifiable risks and benefits of the technology. Its “soft impacts”—effects that cannot easily be quantified, such as changes to experience, habits and perceptions, should receive more attention. (shrink)

Le débat sur les relations esprit–cerveau a été centré sur des questions relatives au libre arbitre. J’examine ce débat et conclus que les neurosciences n’ont pas de raisons méthodologiques, ontologiques ou théoriques convaincantes, pas plus que de raisons empiriques, pour rejeter la notion de libre arbitre. Parallèlement, je reconnais que la question est très controversée, à la fois en science et dans la société. Le problème se situe dans l’incompatibilité entre notions scientifiques du cerveau et notions pré-scientifiques de l’esprit. Par (...) conséquent, je propose d’envisager les relations esprit–cerveau sur une base empirique plus profonde et incontestée que l’on peut trouver dans la psychophysique. Je discute deux cas dans lesquels le contenu d’une expérience psychophysique, et sa dynamique, correspondent à la dynamique cérébrale. Dans un cas, la correspondance est une identité de type; dans l’autre, il s’agit d’une instanciation multiple : plusieurs types radicalement différents d’activités donnent lieu à la même dynamique perceptive. Ces deux exemples illustrent que, bien que l’identité des types soit possible, elle peut être souvent hautement contextualisée. Par conséquent, on doit appréhender les relations esprit–cerveau une par une plutôt que les fonder d’un seul bloc. (shrink)

This paper claims that the standard characterization of the motivational role of belief should be supplemented. Beliefs do not only, jointly with desires, cause and rationalize actions that will satisfy the desires, if the beliefs are true; beliefs are also the practical ground of other cognitive attitudes, like imagining, which means beliefs determine whether and when one acts with those other attitudes as the cognitive inputs into choices and practical reasoning. In addition to arguing for this thesis, I take issue (...) with Velleman's argument that belief and imagining cannot be distinguished on the basis of motivational role. (shrink)

This intervention study examined the effects of a career crafting training on physicians' perceptions of their job crafting behaviors, career self-management, and employability. A total of 154 physicians working in two hospitals in a large Dutch city were randomly assigned to a waitlist control group or an intervention group. Physicians in the intervention group received an accredited training on career crafting, including a mix of theory, self-reflection, and exercises. Participants developed four career crafting goals during the training, to work on (...) in the subsequent weeks, after which a coaching conversation took place over the phone. Physicians in the control group received no intervention. A pre- and post-test 8 weeks later measured changes in job crafting and career self-management and employability of 103 physicians that completed the pre- and post-test. RM ANOVAs showed that the intervention enhanced perceptions of career self-management and job crafting behavior to decrease hindering job demands. No support was found for the effect of the intervention on other types of job crafting and employability. This study offers novel insights into how career crafting can be enhanced through training, as this is the first empirical study to examine a career crafting intervention. HR managers can use the outcomes to develop tailored career policies and career development practices. (shrink)

I argue that Van der Velde and I agree on two fundamental issues surrounding the vision-related binding problem and recent solutions that have been offered: (1) that tagging theories fail to account for object feature binding in visual consciousness and (2) that feedforward-feedback processes in the visual cortical hierarchy play a role in generating a feature-unified object of visual consciousness. Van der Velde develops and discusses an important objection to tagging theories that could help to strengthen my critique of neuronal (...) synchrony (and other tagging theories) and then argues that the cognitive subject makes no explanatory contribution to the unity of an object’s features in visual consciousness. These issues are discussed in turn. By contrast, Van Leeuwen takes a more critical approach to my target article. A two-fold response to Van Leeuwen is offered: first, the root of Van Leeuwen’s perplexity is uncovered and then some specific objections that Van Leeuwen poses to my critique of neuronal synchrony, as a purported solution of the object feature binding problem, are addressed. Although sexual difference is widely regarded as the concept that lies at the center of Luce Irigaray's thought, its meaning and significance is highly contested. This dissensus, however, attests to more than merely the existence of a recalcitrant conceptual ambiguity. That is, Irigaray's discussion of sexual difference remains fraught not because she leaves this concept undefined but because the centrality of sexual difference in fact marks a complex and unstable nexus of phenomena that shift throughout her work. Consequently, if Irigaray (...) is indeed the preeminent thinker of sexual difference, this is not in virtue of her recurrent appeal to a monolithic, readily digestible concept but rather somehow despite the absence of precisely this gesture. In this paper, I will attempt to elucidate the peculiar preeminence of sexual difference in Irigaray's work by identifying her persistent, though largely unexamined, commitment to transcendental phenomenology. Indeed, I attempt to show that the complex of phenomena of sexual difference emerges in L'oubli de l'air and The Way of Love as a modulation of Heidegger's own revision of transcendental phenomenology. In this sense, the peculiar preeminence of sexual difference does not mark the centrality of a concept but Irigaray's amplification of this Heideggerian gesture. (shrink)

In spite of the seminal work A Philosophical Basis of Medical Practice, the debate on the task and goals of philosophy of medicine still continues. From an European perspective it is argued that the main topics dealt with by Pellegrino and Thomasma are still particularly relevant to medical practice as a healing practice, while expressing the need for a philosophy of medicine. Medical practice is a discursive practice which is highly influenced by other discursive practices like science, law and economics. (...) Philosophical analysis of those influences is needed to discern their effect on the goals of medicine and on the ways in which the self-image of man may be changed. The nature of medical practice and discourse itself makes it necessary to include different philosophical disciplines, like philosophy of science, of law, ethics, and epistemology. Possible scenario's of euthanasia and the human genome project in the USA and Europe are used to exemplify how philosopy of medicine can contribute to a realistic understanding of the problems which are related to the goals of medicine and health care. Three puzzles about self-deception make this mental phenomenon an intriguing explanatory target. The first relates to how to define it without paradox; the second is about how to make sense of self-deception in light of the interpretive view of the mental that has become widespread in philosophy; and the third concerns why it exists at all. In this paper I address the first and third puzzles. First, I define self-deception. Second, I criticize Robert Trivers’ attempt to use adaptionist evolutionary psychology (...) to solve the third puzzle (existence). Third, I sketch a theory to replace that of Trivers. Self-deception is not an adaptation, but a spandrel in the sense that Gould and Lewontin give the term: a byproduct of other features of human (cognitive) architecture.Self-deception is so undeniable a fact of human life that if anyone tried to deny its existence, the proper response would be to accuse this person of it. When are we morally obligated as a society to help the homeless, and is coercive interference justified when help is not asked for, even refused? To answer this question, we propose a comprehensive taxonomy of different types of homelessness and argue that different levels of autonomy allow for interventions with varying degrees of pressure to accept help. There are only two categories, however, where paternalism proper is allowed, be it heavily qualified. The first case is the homeless person with severely (...) diminished autonomy as a result of mental illness, and the second case is the homeless person who runs a risk of serious and imminent harm to self. In the first case, namely, that of soft paternalism, we argue that coercive intervention in the case of a refusal to accept help should be focused on the provision of housing that meets basic needs—needs that we outline in the article. In the case of imminent and severe harm to self, the case of hard paternalism, we argue that forced intervention can only be allowed if it is temporary and local, namely focused on getting someone out of harm’s way. In an earlier issue, I argue (2014) that psychology and epistemology should distinguish religious credence from factual belief. These are distinct cognitive attitudes. Levy (2017) rejects this distinction, arguing that both religious and factual “beliefs” are subject to “shifting” on the basis of fluency and “intuitiveness.” Levy’s theory, however, (1) is out of keeping with much research in cognitive science of religion and (2) misrepresents the notion of factual belief employed in my theory. Max Müller, the Sanskritist and foundational pioneer of comparative religion, and the two men’s contrasting relations to F.W.J. Schelling; and finally (...) examines the interest taken in Kierkegaard by William James, Max Weber, and Gerardus van der Leeuw as early twentieth-century contributors through three different disciplines to the study of comparative religion. The recent claiming of Simone de Beauvoir's legacy by French feminists for a policy of assimilation of Muslim women to Western models of self and society reduces the complexity and richness of Beauvoir's views in inacceptable ways. This article explores to what extent a politics of difference that challenges the ideals and political strategies of abstract liberalism can be extracted from and legitimized by the philosophies of Simone de Beauvoir and Jean-Paul Sartre. Without assuming their thought is identical, we can (...) read them as elucidating each other and as implicitly exposing weak and strong points in their respective philosophies on ethnocultural relations and social identities. Richard Sennett can be interpreted as one of the more robust representatives of a current critique with regard to ethnic communities in urban areas, namely, that such ethnic enclaves are a proof of urban disintegration and failing citizenship. Firstly, I take issue with Sennett’s assumption that there is an inherent tension between in-group solidarity and the ability to deal with members of perceived out-groups. Secondly, instead of simply cutting citizens off from the wider public sphere and leaving them politically ineffective, (...) as Sennett argues, urban communities can be instrumental in a struggle for emancipation and equal rights. Thirdly, a sense of belonging is a basic aspect of human well-being. Urban villages or ethnic clusters are able to provide this. Therefore, as long as a local identity is not exclusive or the effect of involuntary segregation, ethno-cultural concentrations in the city are acceptable from the moral-political point of view. This article defends the ethical importance and contemporary relevance of Beauvoir’s 1945 play, Useless Mouths. While the core of Beauvoir’s moral philosophy is already present in this play, she develops it not as a universal maxim but in terms of a historically grounded struggle against exploitation. A Marxist-feminist reading of Beauvoir’s play reveals the moral problem immanent to capitalist modernity: despite the ideals of liberal morality, human life is ontologically worthless.

The moral justification of Will Kymlicka's theory of minority rights is unconvincing. According to Kymlicka, cultural embeddedness is a necessary condition for personal autonomy (which is, in turn, the precondition for the good life) and for that reason liberals should be concerned about culture. I will criticize this instrumentalism of social attachments and the moral monism behind it. On the basis of a modification of Axel Honneth's theory of recognition, I will reject the false opposition between the instrumental value and (...) the intrinsic value of culture. Honneth makes a distinction between three types of recognition: (1) love; (2) respect; and (3) social esteem. Recognition of cultural difference is situated in the third sphere. But the logic of a recognition of cultural difference also demands a non-evaluative recognition, a respect for difference. Difference-respect cannot be reduced to the recognition of personal autonomy or to the recognition of a culture ‘as such’. Difference-respect is concerned with a formal recognition of difference, namely the recognition of a culture's intrinsic value for the other.
